The facility failed to maintain accurate and complete records for a resident, identified as Resident #22, who did not have an inventory sheet in her record. This deficiency was discovered through resident and staff interviews, clinical record reviews, and policy reviews. Resident #22, who had a BIMS score indicating normal cognitive function, reported losing a cell phone after returning from the hospital. The lack of an inventory sheet was confirmed by the Regional Consultant and acknowledged by the Administrator, who stated that without it, the facility could not verify the resident's belongings or determine the validity of claims about missing items.
