Staff members reported using gloves and masks but could not recall wearing gowns or seeing EBP signage. The Director of Nursing confirmed the absence of EBP documentation for R1. These findings indicate a failure to adhere to the facility's EBP policy, resulting in inadequate infection prevention measures for residents with pressure ulcers.
