The Assistant Director of Nursing (ADON) initially claimed that the notice was uploaded to the electronic medical record (EMR), but upon review, no such notice was found. The Director of Nursing (DON) later provided a hard copy of the notice with R62's signature, indicating it was included in the transfer packet given to emergency medical services (EMS) for the hospital. However, the DON could not confirm that R62 received a written copy of the notice, highlighting a failure in ensuring the resident was properly informed as per the facility's policy.
