Summary

The facility failed to ensure that assistive devices, specifically wheelchairs, were maintained and free of hazards for three residents. Observations revealed that the wheelchairs of these residents had cracked armrests with exposed foam and were dirty with dried food substances. These residents were cognitively severely impaired and unable to make decisions for themselves, relying on the facility for their care and safety.
