Summary

The facility failed to ensure the reasonable accommodation of resident needs and preferences for two residents, as evidenced by the improper placement of call lights. Resident 1, who has severe cognitive impairment and is fully dependent on staff for assistance, was observed with their call light on the floor at the foot of the bed, under a trash can, and out of reach. The Director of Nursing acknowledged that both residents could utilize their call lights if they were within reach, indicating a failure to accommodate the residents' needs and preferences adequately.
