Summary

The facility failed to ensure timely notification of a change in condition for a resident, identified as Resident #45, to both the resident's primary care physician and responsible party. Despite this policy, the responsible party, identified as Family Member #672, reported not being informed of the resident's condition changes, including incontinence and the need for incontinence briefs.
