Staff were aware of the residents' behaviors but failed to implement or document effective interventions. The facility's policies on fall management and elopement risk were not adequately followed, resulting in repeated incidents and potential harm to residents. The lack of appropriate supervision and intervention for these residents demonstrated a failure to maintain a safe environment, as required by regulatory standards.
