The LPN acknowledged the expectation to document completed wound care and notify the physician if treatments were not documented. The DON and administrator both expressed that staff should follow physician orders and document treatments, but they were unaware of the missing documentation. The facility had interim nursing leaders during the period of the missing treatments, which may have contributed to the oversight.
