Summary

The facility failed to initiate appropriate interventions to prevent resident-to-resident abuse involving two residents, R1 and R2, in the Memory Care Unit. Further complicating the situation, R1 was moved to a non-secure unit on 12/19/24, which was unsuccessful due to R1's elopement risk, leading to R1's return to the Memory Care Unit on 12/28/24. No interventions were implemented to keep R1 away from R2 after R1's return, leaving R2 vulnerable to further interactions. The facility's failure to implement and document effective interventions and monitoring contributed to the ongoing risk of resident-to-resident abuse.
