The DON confirmed that the resident had orders dated 1/16/26 for OT, PT, and speech therapy evaluations, which were acknowledged by the primary care physician on 1/21/26, and that these ordered evaluations were not completed as directed by the physician.

PT screened the resident two days after admission and identified the need for a standard wheelchair and two-wheeled walker with assistance of one staff for transfers. The PT evaluation and plan of treatment, completed three days after admission, ordered PT five times per week for four weeks, including therapeutic exercises, neuromuscular reeducation, gait training, group procedures, therapeutic activities, and wheelchair management training.
