The policy indicated that safety risks and environmental hazards should be identified on an ongoing basis, and the Quality Assurance and Performance Improvement (QAPI) team should review safety and incident/accident data. However, the facility did not know what caused the resident's injuries and could not address the specific problems that led to the bruises and swelling. The Director of Nursing acknowledged that the facility should have provided constant supervision to assist the resident in ambulation and prevent the incidents that occurred.
