The facility failed to establish and maintain an effective infection prevention and control program, as evidenced by the actions of CNA A, who did not perform hand hygiene after direct contact with four residents while serving meals. This lapse in protocol was observed during meal service on Hall 300, where CNA A interacted with residents without using gloves or sanitizing hands between each resident interaction. Resident #1, a male with moderate cognitive impairment and diagnoses including anemia, hypertension, and heart failure, required assistance with activities of daily living. The facility's policy emphasized hand hygiene as the primary means to prevent infection spread, requiring staff to sanitize hands between each tray service. Despite receiving training, CNA A did not adhere to these protocols, leading to the identified deficiency.
