Similarly, R20 experienced a change in condition and was transferred to the emergency room, later being admitted to the hospital with a complicated urinary tract infection. The surveyor reviewed R20's records and found no evidence that R20's representative was notified of the bed hold policy at the time of hospitalization. The Social Services Director was interviewed and reported being unable to locate the bed hold notifications for the hospitalizations of both R28 and R20, indicating a lapse in the facility's adherence to its own policy.
