Summary

The surveyor's review of R2's medical and treatment records indicated that the resident received medications for anxiety and dementia, but there were no adjustments made despite the increased frequency of behaviors. Interviews with staff confirmed that R2's behaviors were not effectively managed, and the resident continued to pose a risk to themselves and others. The facility's failure to implement and document appropriate interventions and supervision for R2 resulted in a deficiency in providing a safe environment for all residents.
