The facility's policy required the use of N95 masks and other PPE for confirmed COVID-19 cases, but this was not followed. The facility's approach to ending transmission-based precautions was also inadequate. The Medical Director confirmed that the facility ended precautions after one negative test, contrary to CDC guidelines that require two consecutive negative tests. Additionally, there was no dedicated staffing for affected units, leading to potential cross-contamination. The facility's failure to follow recommended testing and isolation protocols, along with inconsistent use of PPE, contributed to the uncontrolled spread of COVID-19 among residents and staff.
