The physician ordered a continuous tube feeding of Osmolite 1.5 Cal at 55 ccs per hour with 200 ccs of sterile water every four hours for hydration, along with a puree diet with thin liquids. Additionally, dysphagia therapy was ordered to be provided 3 to 5 times a week for 4 weeks to help the resident transition to a mechanical soft diet. However, the facility did not provide evidence that the ordered dysphagia therapy was administered to advance the resident's diet.
