Summary

The facility failed to develop and implement timely interventions after a resident fall, affecting one resident out of three reviewed for falls. The resident, who had a mild cognitive impairment and multiple diagnoses including conversion disorder, intellectual disabilities, and dementia, experienced two falls while attempting to go to the bathroom independently.
