The facility was found to have several deficiencies during the surveys conducted, which included issues with accident hazards and bedrail safety. Specifically, the facility failed to maintain hot water temperatures within the safe standard of 110 degrees Fahrenheit, posing a risk of serious harm to residents. This lack of oversight and failure to adhere to regulatory requirements contributed to the facility's inability to use its resources effectively and efficiently to ensure the highest practicable well-being of its residents.
