The Maintenance Director was not responsible for the CAT phones' maintenance, and the DON was unaware of the extent of the call light delays. The Regional Director expected call lights to be answered according to policy, but the system's deficiencies prevented this. The facility's failure to ensure a functioning call light system led to significant delays in responding to residents' needs, as evidenced by the survey findings.
