On several occasions, the schedule lacked evidence of nurse coverage for specific halls, leaving residents without proper care. The RDCO acknowledged the documentation issues and the need for re-education of the nursing staff, but the facility did not have a formal policy for staffing, contributing to the ongoing deficiencies.
