The facility failed to ensure that the Pre-Admission Screen Resident Review (PASRR) recommendations were completed for a resident with a qualifying diagnosis and disruptive behaviors. The facility's administrator was unaware of R4's PASRR requirements, and there was no policy in place for managing residents with behavioral health issues.
