Summary

Hospital discharge instructions specified the use of Aquacel dressings and daily dressing changes, but these orders were not transcribed into the facility's electronic medical record (EMR) until two days after readmission. During the survey, it was observed that the resident's surgical site was open to air without a dressing, contrary to the hospital's discharge instructions. The LPN responsible for the resident acknowledged the lack of completed dressing changes and the absence of physician orders for the surgical site care upon readmission. The Director of Nursing confirmed that the admission nurse should have reviewed and transcribed the hospital's discharge orders into the EMR immediately upon the resident's readmission.
