The staff acknowledged that not implementing care plans could lead to resident harm, including falls, disease exacerbation, and weight fluctuation. The facility's policy emphasized the importance of developing and implementing comprehensive care plans to meet residents' needs, but these were not effectively executed for the residents in question.
