The facility failed to adhere to professional standards for food service safety, affecting 38 out of 40 residents. The facility's policies on dishwashing machine use and sanitization were not followed, as evidenced by the lack of training for food service staff and the failure to maintain cleanliness in the kitchen and dining areas. The dishwashing machine's sanitizer concentration was below the required levels, and the facility's failure to maintain sanitary conditions could place residents at risk for foodborne illnesses.
