Summary

The facility failed to maintain an effective infection prevention and control program, as evidenced by multiple instances of non-compliance with Enhanced Barrier Precautions (EBP) and hand hygiene protocols. For Resident 5, who was on EBP due to colonization with multidrug-resistant organisms (MDROs), Certified Nursing Assistants (CNAs) did not wear gowns during high-contact care activities, and a Registered Nurse (RN) failed to perform hand hygiene between glove changes during wound care. Additionally, the RN did not wear a gown while providing care, contrary to the facility's policy and CDC guidelines. Resident 1, also on EBP, experienced similar lapses in infection control.
