The facility's inaction and failure to adhere to its own policies and procedures regarding elopement prevention contributed to the resident's ability to elope on two occasions. The lack of adequate supervision and proper placement of the Wanderguard created a reasonable likelihood for serious harm to the resident. The facility's deficient practice was identified as immediate jeopardy, indicating a situation in which the facility's noncompliance with one or more requirements of participation has caused, or is likely to cause, serious injury, harm, impairment, or death to a resident.
