The facility failed to establish and maintain an effective infection prevention and control program, as evidenced by the lack of proper transmission-based precautions for a resident diagnosed with methicillin-resistant Staphylococcus aureus (MRSA). The resident, who had a Charcot foot wound infected with MRSA, was not properly identified with a transmission-based precautions sign outside their room. Interviews with the therapy staff revealed a misunderstanding of the requirements for wearing gowns during physical contact with the resident. The Director of Nursing confirmed that therapy staff should have worn gowns to prevent accidental exposure, especially given the resident's wound and drainage issues.
