During a tour of the kitchen, it was observed that numerous food items, including brownie mix, canned vegetables, and spices, lacked proper labeling with dates of receipt or opening. This lack of labeling contravenes the facility's policy, which requires all food items to be labeled with content, received date, and either manufacturer information or relevant disposal dates. The Dietary Manager confirmed that items were not dated upon receipt into the kitchen, and opened packages were not dated with an open date. Additionally, during a tray line observation, an employee was seen handling food in an unsanitary manner. The employee retrieved a package of sliced cheese from the refrigerator and then used the same gloves to handle bread and make a sandwich without changing gloves or washing hands. This action was confirmed by the employee, who acknowledged the failure to change gloves and wash hands after touching the refrigerator door handle. Interviews with facility staff, including the Acting NHA/Chief Performance Improvement Executive and the Chief Operating Officer, revealed that the facility's current practice of not carrying over dates from bulk storage to the kitchen contributed to the deficiency. The lack of dating at the time of receipt or purchase prevented the facility from utilizing governmental guidelines to determine expiration dates, further exacerbating the issue.
