The facility failed to maintain sufficient and competent staff on the first floor, affecting several residents, including Resident #15, who was found in a state of neglect. Resident #15, with a history of spastic hemiplegia, hypertension, and osteoarthritis, was not provided with incontinence care from 1:00 A.M. until 11:26 A.M. on the day of the survey. The resident was found lying in bed with a strong urine odor, soaked incontinence products, and stained bedding, indicating prolonged neglect. The staff, including CNAs and LPNs, were unaware of the changes in assignments, leading to a lack of care for Resident #15.
