Similarly, Resident #71's records showed discrepancies in medication administration. The MAR recorded the administration of a laxative, but observations indicated that the medication was not offered. Nurse #3 mistakenly believed the medication had been discontinued and documented its administration inaccurately. Interviews with the resident and the DON confirmed the error, highlighting the need for accurate documentation and proper communication regarding medication orders.
