The staff on duty during the incidents did not follow the protocol of notifying the medical provider or documenting the refusals in the Medication Administration Record. Additionally, the receiving unit was not informed of the reasons for Resident #3's transfer, which contributed to the subsequent altercation with Resident #4. This series of inactions and communication failures led to the deficiency in ensuring residents' right to be free from abuse.
