Interviews with facility staff revealed confusion and lack of clarity regarding the responsibility for completing and following up on bed hold forms. The Director of Nursing and the Regional Corporate Nurse indicated that charge nurses were responsible for completing the forms, and the Social Services Designee was expected to ensure their completion and notify the responsible party if the resident was unable to sign. However, the bed hold forms were not completed for Resident #5, as the family reportedly declined the bed hold, although the DON later stated that the resident would be the one to decline it.
