The facility failed to provide adequate behavioral health interventions for a resident who exhibited behaviors that escalated to a catastrophic reaction, resulting in the use of an antipsychotic medication injection. The resident, who had a history of bipolar disorder, psychosis, major depressive disorder, and other mental health conditions, was observed to have verbal outbursts and wandering behaviors.
