Summary

The facility staff failed to meet professional standards of care for Resident #31 by not providing the prescribed treatment as ordered. The resident, who has moderate cognitive impairment, lower extremity impairment, and diagnoses of Parkinson's and Diabetes, had a physician's order for Tubi grip to be applied to both lower extremities every shift and removed at bedtime for generalized edema. However, observations on multiple occasions showed the resident without the Tubi grips. Interviews with various staff members, including the social services director and the assistant director of nursing, highlighted a lack of awareness regarding the inconsistencies in code status documentation. The social services director, responsible for maintaining and updating advance directives, admitted to not being aware of the discrepancies and acknowledged that audits were not conducted frequently enough to ensure accuracy. The administrator also expressed concern that inconsistent records could lead to residents' wishes not being upheld in an emergency.
