The EMR showed no documentation of a written bed hold notice being provided at the time of transfer. Interviews with the DON and the Administrator further confirmed that the facility did not provide written bed hold notices to residents or their representatives, indicating a systemic issue in adhering to the facility's policy and state requirements.
