Summary

Observations revealed that staff did not don PPE, such as gloves and gowns, when transferring the resident from a wheelchair to the bathroom, despite a sign on the resident's door indicating the requirement for PPE during high-contact activities. Staff members, including a trained medication aide and nursing assistants, were seen handling the resident without gloves and gowns, contrary to the facility's EBP policy.
