This failure had the potential to adversely affect all 46 residents residing in the facility. Staffing issues were prevalent, with insufficient staff to transfer residents according to their care plans and to provide appropriate supervision to prevent resident-to-resident abuse. The facility's assessment identified a high staff turnover rate and failed to implement interventions to improve this rate or ensure safe resident care. The QAPI meetings lacked goals and measurable actions regarding abuse, staffing concerns, accidents, and infection prevention, indicating a systemic issue in addressing and rectifying these deficiencies.
