Similarly, Resident #2, who was cognitively intact and had diagnoses including Parkinson's, seizures, bipolar disorder, and schizophrenia, was transferred to the hospital six times in 2024 without receiving a bed-hold form. During interviews, both the Director of Nursing and the Unit Manager acknowledged that bed-hold notifications should be provided with each hospital transfer. However, the forms were not found in the resident's medical records or the facility's filing system. Medical Records staff also confirmed that no bed-hold forms were uploaded into the electronic medical records for Resident #2.
