Deficiency in Providing Palatable Hot Beverages

Majestic Care Of Battle CreekBattle Creek, Michigan Survey Completed on 10-30-2024

Summary

The facility failed to provide hot liquids at a palatable temperature to several residents, as observed during a group interview and individual resident survey. Five out of eight residents in a group interview expressed frustration with the temperature of beverages, noting that coffee and tea were consistently cold, and hot cocoa was not hot enough to dissolve properly. One resident, who was cognitively intact and had clear speech and adequate hearing, reported dissatisfaction with the cold coffee, which led to the resident's spouse bringing in coffee from outside. This resident had filed a grievance about the coffee temperature, but no changes were made by the facility. During a kitchen tour, it was found that the facility maintained a temperature log for hot liquids between 120F and 140F, but the Dietary Manager was unsure of the policy's origin. The Nursing Home Administrator and Director of Nursing were also unfamiliar with the hot liquid policy. A review of the facility's documentation revealed a past incident where a resident was burned by hot liquids, leading to a corrective plan that included evaluating residents for hot liquid risks. However, the facility's policy stated that residents should not be restricted from hot beverages, and interventions should be implemented to minimize burn risks, such as maintaining a serving temperature of not more than 180 degrees Fahrenheit and using protective measures.

Short Summary

A facility failed to ensure meals were palatable and served at proper temperatures. Several residents reported that food was cold or could be warmer, and 15 confidential residents said meals were cold most of the time across all meal forms. F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Pureed diet food was not prepared according to the facility recipe for two residents. A dietary aide processed smothered pork chops and gravy in a food processor without referring to the available recipe, while stating he knew what to add to thin or thicken food as needed. The dietary manager said she was still educating staff on proper procedures, and the facility policy required puree foods to be smooth, soft, and homogeneous with specific preparation guidelines.

Short Summary

Cold Food Served on Tray Line and at Point of Service: Hot menu items were observed at low temperatures on the tray line, with pureed items tasting lukewarm and not palatable. A test tray sent to the unit was later measured at 115 degrees F for pork, 97 degrees F for green beans, and 100 degrees F for rice, and both the surveyor and the Culinary Development Specialist described the food as cold, bland, and not hot or warm. Residents reported the food is always cold, sometimes requires reheating that staff are too busy to provide, and that they buy outside food or snacks because the kitchen meals are not hot enough.
