A policy addressing annual employee training was requested but not provided prior to exit.

The Business Office Manager confirmed the personnel records for Employee #4, #10, #11, #12, #13, #15, #16, and #17 lacked documented evidence of Communication training completed upon hire and annually. The facility policy titled, Nursing Personnel Education and Training, published 11/2016, stated education for new employees and contract staff is provided as part of orientation and scheduled annually, including Communication Training.
