The facility failed to provide appropriate respiratory care for three residents, as observed during a survey. Resident #21's nebulizer mask was repeatedly found stored improperly on a bedside table next to a soiled cushion, exposing it to potential contamination. This resident had a history of chronic obstructive pulmonary disease, chronic pulmonary edema, and other respiratory conditions, requiring regular nebulizer treatments.
