Summary

The facility failed to ensure that the fingernails of two residents were clean and trimmed, as required by their care plans and facility policy. Resident #2, who has a history of dementia and cognitive impairment, was observed with black and curled fingernails. Despite being dependent on staff for all activities of daily living (ADLs), the necessary grooming was not provided.
