Failure to Provide Salt Alternatives for NAS Diet

Riviera Healthcare CenterPico Rivera, California Survey Completed on 11-15-2024

Summary

The facility failed to provide a salt alternative seasoning for a resident on a no added salt (NAS) diet, leading to complaints of unappetizing and flavorless meals. This deficiency was identified for a resident who had been admitted with diagnoses including heart failure, stroke, and hypertension. Despite having the capacity to understand and make decisions, the resident experienced consistent weight loss over several months, which was not planned or intentional. The resident resorted to using high-sodium hot sauce packets to add flavor to her meals, unaware of their sodium content, due to the unavailability of salt alternatives. Interviews with the Dietary Supervisor (DS) revealed that the facility did not have salt alternatives available, and the DS had not ordered them from a more expensive vendor despite knowing their importance for residents on a NAS diet. The DS acknowledged that decreased palatability could lead to reduced food intake and weight loss. The Licensed Vocational Nurse (LVN) responsible for checking meal trays admitted to not checking for high-sodium condiments, which could exacerbate the resident's medical conditions. The facility's Registered Dietician confirmed that the resident was not on a planned weight loss regimen, and the Registered Nurse (RN) noted that the facility should have addressed the resident's oral intake and food palatability. The facility's policies indicated that care planning for weight loss should be a multidisciplinary effort, including input from the RD, and that documenting meal refusal due to non-compliance was inappropriate. However, these policies were not effectively implemented, contributing to the resident's undesirable weight loss and dissatisfaction with meals.

D
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Two residents were served microwave-reheated items without a temperature check before service. A staff member reheated almond milk and chicken noodle soup, placed both on the counter, and they were later measured at 182 F and 190.4 F, respectively. Staff stated the kitchen checked temperatures, but the items were served before confirming they had cooled to a safe temperature.

Short Summary

A resident with cognitive impairment and a mechanically altered diet was left sitting at the dining table with an uncovered lunch tray for nearly 45 minutes without staff cueing or feeding assistance, despite needing supervision and help with meals. When an NA finally began feeding her, the meal items were found to be lukewarm or cold, and neither the NA nor the dietary manager reheated the food or replaced the tray.

Food Portions Not Measured Per Recipe
F
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food portions were not served per recipe directions when a cook prepared sandwiches, pureed items, cake, and side items by estimation instead of measured amounts. The cook used tongs to add unmeasured meat, onions, and cheese to buns, served visibly different cake portions, and confirmed there were not enough pickles for all residents. The cook stated the portions were “eye balled” and that the spoons were inconsistent.

E
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food was not prepared to preserve nutritive value for 233 of 237 residents receiving kitchen-prepared meals. E
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food and beverages were not served at proper temperatures and were described as unpalatable during a lunch meal. Tray line issues included no plate warmer in use, cold drinks left unrefrigerated, and delayed tray delivery, and the test tray showed meat loaf, mashed potatoes, vegetables, milk, and coffee at suboptimal temperatures. Residents and an LPN reported the meal was cold, bland, dry, tough, or tasteless, and the FSD and RD gave differing expectations for hot and cold food temperatures.
