Summary

The facility failed to protect a resident from sexual abuse by a visitor, specifically a family member who was also the resident's power of attorney (POA). However, on October 14, 2024, there was no evidence that the family member was supervised during his visit with the resident. The resident later reported to hospital staff that she had been sexually assaulted by the family member/POA during the visit. Interviews with facility staff revealed that the family member/POA was not consistently supervised during visits, and there was confusion among staff about where the visits took place and whether safety precautions were in place. The facility's policy on abuse and visitor management required that residents be protected from harm and that access by alleged perpetrators be restricted during investigations. However, these policies were not effectively implemented, leading to the resident's exposure to further abuse.
