Summary

The facility failed to provide necessary behavioral health care and services for a resident with a history of generalized anxiety disorder, depression, and opioid dependence. Upon readmission, the resident experienced a decline in cognition and overall medical condition, as noted in the Minimum Data Set (MDS) assessment. Despite these changes, the facility did not complete a social services assessment for the resident's change in condition.
