Summary

The facility failed to ensure that a resident maintained acceptable nutritional status, resulting in significant weight loss. Resident 298, who had diagnoses including dementia and acute kidney failure, was admitted with a care plan addressing potential nutrition deficits. Despite having orders for a fortified diet and nutritional supplements, the resident experienced a 10.41% weight loss from June 2 to July 11, 2024.
