The DON confirmed the absence of documentation for non-pharmacological interventions or daily behavior monitoring. Similarly, Resident #21's records lacked documentation of behaviors or non-pharmacological interventions before administering Ativan as needed. The facility's policy stated that psychotropic drugs should only be given when necessary and beneficial, with monitoring and documentation of the resident's response, which was not adhered to in these cases.
