For instance, on 07/19/24, the Posted Nurse Staffing indicated 11 Nursing Assistants (NAs) worked, while the actual schedule showed only 10 NAs. Similar inconsistencies were noted on 07/21/24, where the number of NAs, Licensed Practical Nurses (LPNs), and Registered Nurses (RNs) listed on the Posted Nurse Staffing did not match the actual staff working according to the schedule. These discrepancies were repeated on 07/22/24 and 07/23/24, with variations in the reported and actual numbers of NAs, LPNs, and RNs working different shifts. Interviews with the Human Resource Coordinator revealed that errors in the staffing sheets were due to counting an unlicensed staff member twice and mixing up dates, leading to incorrect postings. The Coordinator confirmed that the staffing sheets for the specified dates did not match the staff posting sheets. The Administrator also acknowledged the expectation that the daily nurse staff sheets and assignment sheets should accurately reflect the correct number of staff working, indicating a failure in maintaining accurate staffing records.

During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
