The facility staff failed to consistently document the Activities of Daily Living (ADL) status and care provided to a resident, as required by facility policies and procedures. Despite this expectation, there were numerous instances where documentation was incomplete, with no explanation provided for the blanks in the resident's DSR. The facility's policies on nursing documentation and ADLs, revised in May 2023, emphasized the importance of timely and accurate documentation reflective of the care provided. However, the failure to adhere to these policies resulted in the identified deficiency.
