The facility's policy requires that the Bed Hold Policy Notice & Authorization form be given to the resident and their representative prior to transfer, with the representative's copy delivered within 24 hours if not present. Despite this policy, the facility did not have documentation to show compliance for the specified dates. The surveyor's findings were communicated to the Licensed Nursing Home Administrator (LNHA) and Director of Nursing during the exit conference, and the facility did not provide additional information or refute the findings.
